Counted Out: How America's Occupational Disease Surveillance Architecture Fails Its Most Vulnerable Workers

A Surveillance System Built on Incomplete Data
When epidemiologists attempt to quantify the burden of occupational disease in the United States, they encounter a fundamental methodological problem: the data architecture was never designed to provide a complete picture. The Bureau of Labor Statistics Survey of Occupational Injuries and Illnesses (SOII), the primary national instrument for workplace health tracking, relies on employer self-report and excludes self-employed workers, farms with fewer than 11 employees, and several other significant labor categories. The Occupational Safety and Health Administration (OSHA), for its part, collects inspection and violation records that rarely interface in any systematic way with state or federal epidemiological databases. What emerges is not a coherent surveillance network but a patchwork of disconnected data streams, each capturing fragments of a much larger and more troubling public health reality.
The consequences of this fragmentation extend well beyond bureaucratic inconvenience. When occupational exposure data fails to flow into epidemiological tracking systems, researchers and clinicians lose the capacity to identify emerging disease clusters, characterize exposure-response relationships, or intervene before acute incidents escalate into chronic health crises. In sectors where chemical, thermal, and ergonomic hazards are concentrated—agriculture, construction, and manufacturing in particular—the failure to detect and communicate risk in real time translates directly into preventable morbidity and mortality.
The Undercount Problem: Why Occupational Illness Disappears from the Record
A substantial body of peer-reviewed research has documented the systematic underestimation of occupational illness in administrative data. Studies published in journals including the American Journal of Industrial Medicine and the Journal of Occupational and Environmental Medicine have consistently found that employer-reported illness rates undercount true incidence by factors ranging from two to seven, depending on the industry and condition under examination. Chronic diseases with long latency periods—occupational cancers, silicosis, chronic obstructive pulmonary disease attributable to dust exposure—are particularly susceptible to misclassification or omission, as their etiological link to workplace exposures is rarely captured in routine medical records or death certificates.
Several mechanisms drive this undercount. Workers in precarious employment situations may fear retaliation for reporting illness or injury, a concern that is empirically grounded given documented cases of employer discouragement and underreporting incentive structures tied to safety performance metrics. Physicians treating occupational illness frequently lack formal training in occupational medicine and may not elicit or document workplace exposure histories. State workers' compensation systems, which generate data that could theoretically supplement surveillance, operate under highly variable eligibility criteria and benefit structures, further distorting the epidemiological signal.
Amplified Risk, Amplified Invisibility: Immigrant and Low-Wage Workers
The surveillance deficit is not distributed uniformly across the workforce. Workers in agriculture, meatpacking, residential construction, and light manufacturing—sectors with high concentrations of immigrant labor and low-wage earners—face a compounding disadvantage. They carry elevated occupational exposure burdens while simultaneously being least likely to appear in existing surveillance data.
Undocumented immigrant workers face particularly acute barriers to reporting. Fear of immigration enforcement, language barriers, lack of health insurance, and unfamiliarity with regulatory systems all suppress utilization of formal reporting mechanisms. Research conducted in agricultural communities across California, North Carolina, and Florida has documented elevated rates of pesticide-related illness, heat stroke, and musculoskeletal injury among farmworkers that are almost entirely absent from OSHA records or state illness registries. Seasonal and temporary employment arrangements further complicate attribution: by the time a chronic condition manifests clinically, the worker may have moved, changed employers, or lost contact with the occupational health system entirely.
This invisibility is not incidental. It reflects longstanding exclusions embedded in the regulatory framework itself. The original Occupational Safety and Health Act of 1970 excluded agricultural workers on small farms from OSHA jurisdiction, a carve-out that has persisted for more than five decades and continues to exempt a substantial share of the most hazardous agricultural work from federal oversight.
The OSHA–Epidemiology Disconnect
Perhaps the most consequential structural gap in the current system is the absence of routine, bidirectional data linkage between OSHA's enforcement and inspection records and state and federal epidemiological surveillance infrastructure. OSHA collects substantial information through workplace inspections, employer-submitted 300 logs, and incident investigations. This data, in principle, could serve as a sentinel surveillance mechanism capable of flagging emerging hazard clusters and triggering epidemiological follow-up. In practice, it rarely does.
The Centers for Disease Control and Prevention's National Institute for Occupational Safety and Health (NIOSH) maintains the National Occupational Mortality Surveillance (NOMS) system and the Fatality Assessment and Control Evaluation (FACE) program, but these systems operate largely independently of OSHA's administrative data infrastructure. Attempts to link occupational exposure records with mortality registries or electronic health record systems have been limited in scope and have not been implemented at a scale sufficient to support real-time or near-real-time surveillance.
Several states have developed more integrated occupational health surveillance programs—California's SENSOR program and Washington State's occupational disease reporting system are frequently cited as models—but their coverage is geographically limited and their sustainability depends on variable state-level political and budgetary commitments.
Toward Structural Reform: What an Adequate Surveillance System Would Require
Addressing the occupational health surveillance gap requires intervention at multiple levels of the data infrastructure. At minimum, meaningful reform would involve mandatory electronic reporting of occupational illness and injury to a centralized, standardized national registry with interoperability requirements linking OSHA data to state epidemiological systems. Expansion of OSHA's jurisdiction to cover currently exempted agricultural operations would eliminate one of the most glaring regulatory exclusions. Investment in occupational medicine training for primary care providers, combined with standardized occupational exposure history documentation requirements in electronic health records, would improve case ascertainment at the clinical level.
For populations with documented barriers to formal reporting, community-based participatory surveillance models—piloted successfully in several immigrant worker communities—offer a complementary approach. These programs train workers and community health workers to identify and document occupational illness, creating data streams that can supplement administrative sources.
The epidemiological cost of the current system's inadequacies is difficult to quantify precisely, which is itself a reflection of the problem. Until the United States builds an occupational health surveillance infrastructure capable of seeing all of its workers, the true burden of work-related disease will remain, by design, uncounted.
